Care Management Extender

The Care Management Extender provides care management functions, documentation, referral and linkage, and monitoring/follow-up under direct and indirect supervision. This role involves participating in interdisciplinary treatment planning, providing crisis intervention, and ensuring clients receive coordinated care to achieve their health and recovery goals. The position requires maintaining accurate documentation and facilitating access to medical, behavioral, and social services.
