Care Management Intern

This internship supports the HCBS and ER Departments by assisting care managers with care management and advocacy for managed care enrollees in the LTSS division. The role involves evaluating assessment data, making Level of Care determinations, and collaborating with external agencies to maintain Medicaid eligibility. Interns will also help develop individualized care plans, provide consumer education, and manage enrollment processes.
