Waiver Service Coordinator

This role provides delegated Waiver Service Coordination for MyCare Ohio HCBS Waiver members at the Nursing Facility level of care, ensuring person-centered, high-quality support. The position involves conducting needs assessments, developing service plans, and collaborating with care coordinators and state agencies to manage Medicaid eligibility and care coordination. The coordinator also provides education, crisis intervention, and advocacy for consumers and their families.
