Care Management

The Care Management role delivers care management services to patients enrolled in the clinic's Chronic Care Management (CCM) and Principal Care Management (PCM) programs. This position involves operating care management programs, managing a panel of enrolled patients, and engaging with them monthly according to their clinician’s plan of care. The role works under the supervision of a licensed Physician to ensure high-quality patient care.
