Care Management

The Care Management role delivers care management services to patients enrolled in The Neuron Clinic's Chronic Care Management (CCM) and Principal Care Management (PCM) programs under the supervision of a licensed Physician. This position involves directly managing a panel of complex neurological patients and engaging with them monthly according to their clinician’s plan of care. The role requires maintaining expert knowledge of care management scope of practice and collaborating with internal departments to ensure successful patient care completion.
