Care Manager Assistant PD

The Care Manager Assistant PD serves as a liaison to connect patients and families to the appropriate level of healthcare services to ensure optimal health outcomes. This role collaborates with the care team to identify and reduce barriers to care, assist families in accessing resources, and educate families on the importance of timely care. The position reports to Care Coordination leadership and works closely with the VP of Population Health.
