Care Manager

This role serves as a liaison between members and home and community-based service providers to facilitate quality, cost-effective care and outcomes. The Care Manager provides education, support, and resource coordination to members and their families while maintaining regular communication through phone calls and home visits. The position involves collaborating with internal teams and external stakeholders to ensure continuity of care, resolve care plan variances, and monitor member wellbeing.
