About this role
The Care Navigator is a full-time frontline role supporting Medicare-eligible members in rural Lansing/Ionia/St. Johns counties to achieve their best health. The navigator engages members via phone, video, and in-person visits to connect them with medical, behavioral, and social services, and works with local providers and communities to improve access to care. This position is part of Homeward’s care delivery model focused on value-based outcomes for rural seniors and includes documentation, community engagement, and member coaching.
