The Care Navigator is a member of an Integrated Care team working within an inpatient psychiatric clinic to coordinate and support mental health care. The role involves collaborating with primary care providers, behavioral health clinicians, and psychiatric providers, coordinating referrals, and facilitating access to appropriate services. The Care Navigator may also deliver brief evidence-based interventions and support measurement-based care tracking.
