About this role

The Home Care Assistant provides in-home support to WelbeHealth PACE participants as part of an interdisciplinary care team, helping seniors remain in their homes and communities. This full-time role offers a stable schedule with 2–5 patient visits per day, reimbursement for travel, and opportunities to assist with care planning.
