Case Manager (Bridge Team)

A Case Manager provides direct support and monitoring to adults with severe psychiatric disorders or substance abuse issues to improve their psychological adjustment and quality of life. The role involves home visits, school visits, crisis de-escalation, and collaboration with treatment teams to ensure consumersmaintain the least restrictive environment. Responsibilities include conducting SUN-R assessments, creating Individualized Service Plans (ISP), and linking clients to community resources.
