The Health Systems Navigation (HSN) Case Manager at GLIDE provides comprehensive, client-centered support to individuals who use substances, with a focus on harm reduction and health systems navigation. The role manages a dedicated caseload, develops individualized care plans, and supports participants’ access to primary care, behavioral health, substance use treatment, and HIV/HCV/STI prevention and care. Qualifications
