Peer Navigator

The Peer Navigator connects patients with HCV, HIV, and sexually transmitted infection prevention, testing, treatment, and related services, including substance use disorder treatment and behavioral healthcare. This fully in-person role provides navigation, education, and recovery support primarily from a mobile unit, with additional clinic and community-outreach responsibilities. The position supports patients with insurance coverage, referrals, and follow-up care while maintaining accurate documentation in electronic health records.
