About this role

A Community-Based Case Manager at Highmark Inc. serves as the single point of contact for members to coordinate care across service delivery systems and community supports. This full-time role works directly with members in their homes, nursing facilities, and other community settings for individuals enrolled in DSHP Plus LTSS and DSNP and requires frequent travel within the assigned Delaware territory. The position supports long-term services and supports and facilitates care transitions to maintain members in the least restrictive safe environment.
