Case Manager – Permanent Supportive Housing

The role provides case management services to families experiencing homelessness within the Permanent Supportive Housing Program, assisting them with accessing benefits, coordinating community resources, and supporting landlord relationships to achieve self‑sufficiency and stable housing. The position is based on‑site at Lucy’s Hearth in Middletown, RI, with visits to client apartments across Aquidneck Island and includes rotating on‑call duties.
