Case Manager

This role provides case management services to families experiencing homelessness within the Permanent Supportive Housing Program, focusing on intervention strategies to improve self-sufficiency and secure permanent housing. The position involves assisting clients with accessing mainstream benefits, coordinating with community resources, and supporting landlord relationships. Based on-site at Lucy’s Hearth in Middletown, RI, the role also requires visiting families in apartments across Aquidneck Island and participating in rotating on-call duties.
