Responsibilities

Conduct face-to-face home visits and in-person assessments for members enrolled in MLTSS and D-SNP programs
Develop individualized care plans and coordinate interdisciplinary meetings to ensure safe transitions
Document assessments and interactions accurately in the electronic health record and mentor new staff
