Case Manager

This role involves coordinating Case Management Department activities under the direction of a Case Manager or Social Worker to develop and implement safe, patient-focused discharge plans. The position requires assisting with case management referrals, authorizations, and maintaining Medicaid state submission forms. The ideal candidate will have knowledge of discharge planning and utilization management terminology in both managed and non-managed care environments.
