Case Manager

This role provides addiction intervention, AODA counseling, and recovery support services to pregnant and post-partum mothers and their families, focusing on successful reentry outcomes. The position involves coordinating behavioral health, public agencies, and community resources to assist clients in achieving established goals. It requires working with high-risk caseloads, co-occurring clients, and families both inside prisons and in the community.
