Queens Community House is seeking a participant-centered Case Manager to join the Queens Connect Career Services team, working directly with young adults ages 18–24 to identify barriers and support their educational, career, and personal goals. The role involves developing individualized service plans, coordinating referrals, monitoring participant progress, and providing comprehensive support services to ensure successful outcomes. The Case Manager will also collaborate with internal teams and external partners to connect participants with resources and co-facilitate workshops focused on job readiness and personal development.
