Centers For Independence
Join the team as a Case Manager to support individuals living with severe mental illness and/or substance use disorders in building healthier, more independent lives. You will develop individualized treatment plans, provide supportive counseling, and connect clients with essential community resources and services. This role requires collaboration with an integrated treatment team to coordinate care and advocate for client needs.
