The Case Manager supports individuals living with severe mental illness and/or substance use disorders as they work toward greater stability, independence, and quality of life. This role involves building trusting relationships, developing client-centered treatment plans, and connecting clients with valuable community resources. The position requires collaboration with an integrated treatment team, families, and community providers to advocate for client needs and support their recovery journey.
