Case Manager

The Case Manager assesses clients to determine the appropriate level of care, develops and monitors Individual Service Plans, and connects clients with community resources. This role provides ongoing support and coordination with clients, families, and service providers to ensure person-centered care and promote independent living skills. The position requires maintaining continuity of services, managing documentation, and fostering relationships with community partners.
