The Case Manager serves as a key member of the patient care delivery team in a Magnet Recognized large tertiary academic medical center, utilizing clinical expertise to perform the nursing process within a patient-centered care professional practice model. This role ensures medical appropriateness criteria are met for the level of care provided and collaborates with an interdisciplinary team to implement comprehensive discharge plans that meet the patient's continuing healthcare needs. The position requires maintaining a solid working knowledge of specialized case and utilization management methodologies while functioning within the expectations of a professional nurse to achieve and maintain Magnet Recognition standards.
