Case Manager

A Registered Nurse provides skilled nursing care to patients on an intermittent basis in their place of residence, following physician orders and a plan of care under the supervision of the Clinical Manager. The role involves conducting initial evaluations, assessing patient needs, and coordinating with the interdisciplinary team to develop and revise care plans. The Case Manager also supervises other nursing personnel, completes necessary documentation, and ensures compliance with agency policies and regulatory requirements.
