Case Manager

The Case Manager supports clients experiencing homelessness by providing case management services, linking them to housing and community resources, and assisting with care to promote safe, stable housing and self-sufficiency. This role utilizes a trauma-informed and strength-based approach to build trust, set goals, and help clients overcome challenges while maintaining accurate documentation and collaborating with community partners. The position also involves facilitating groups, providing crisis intervention, and advocating for equitable access to services.
