OHM Case Manager

The OHM Case Manager provides outreach-driven case management services to individuals experiencing homelessness or housing instability, actively engaging with them in the community to complete housing applications and enroll them in benefits. This role supports clients in achieving housing stability and improved health outcomes through trauma-informed street outreach and intensive case management. The position requires developing referral relationships with social service agencies and maintaining accurate case documentation in HMIS.
