Case Manager

The Case Manager provides comprehensive case management services for individuals with substance abuse problems and/or HIV, serving as an advocate for clients and their families. The role involves coordinating health care and psychosocial services, conducting assessments, and developing individualized service plans to ensure continuity of care. Additionally, the position requires facilitating referrals, maintaining accurate documentation, and collaborating with community coalitions to enhance the continuum of care.
