Medical Case Manager

This position provides comprehensive case management services for individuals infected with HIV, serving as an advocate and coordinating access to health care, psychosocial, and supportive services. The role involves conducting initial assessments, developing individualized service plans, and ensuring continuity of care through collaboration with internal teams and external community resources. The Medical Case Manager also manages client data, prepares reports, and ensures compliance with agency standards and grant requirements.
