Case Manager

This role provides case management services for children entering or currently in out-of-home care, focusing on their successful placement, stabilization, and permanency. The position involves coordinating with families, state agencies, and community resources to ensure safety and meet the child's medical, educational, and emotional needs. The case manager is responsible for developing permanency plans, facilitating family meetings, and securing legal permanency for each case.
