Case Manager

EAC Network is seeking a dedicated Case Manager to provide enhanced transition planning and care management services to individuals with substance use disorders being released from local correctional facilities in Suffolk County. The role involves developing reentry service plans, facilitating connections to post-release services such as housing and treatment, and ensuring timely activation of benefits like Medicaid. The Case Manager will work within correctional facilities and maintain ongoing client contact to support successful community reintegration.
