Case Manager

The Case Manager serves on the Prevention team at WestCare Tennessee, delivering quality services to eligible veterans and their families through the Staff Sergeant Parker Gordon Fox Suicide Prevention Grant. This role focuses on suicide prevention, awareness, and linking individuals to community resources within designated counties in Tennessee. The position involves conducting initial contacts, developing wellness plans, and facilitating weekly case management meetings to support veteran well-being.
