Case Manager

The Case Manager is responsible for the case management and navigation of clients enrolled in the Bridges to Success Adult Reentry program, utilizing a Housing First approach to assist formerly incarcerated individuals in achieving housing stability. This role involves coordinating services, managing a caseload of up to 25 participants, and facilitating their reintegration into the community through supportive services and advocacy. The position requires collaboration with various agencies to ensure clients receive necessary health, employment, and housing resources.
