Case Manager

The Case Manager is responsible for the case management and navigation of clients enrolled in the Bridges to Success Adult Reentry program, utilizing a Housing First approach to assist formerly incarcerated individuals in achieving housing stability. This role involves managing a caseload, coordinating supportive services, and advocating for clients to reintegrate into their communities and break the cycle of homelessness and incarceration. The position requires maintaining detailed documentation, collaborating with community resources, and ensuring successful client outcomes through ongoing support and monitoring.
