Case Manager

This role provides case management services to individuals committed to a State Mental Health Treatment Facility, tracking their progress during treatment and post-discharge to ensure treatment engagement, stable housing, and community involvement. The position facilitates communication between treatment facilities and behavioral health systems to coordinate appropriate services and support client recovery. Responsibilities include developing psychosocial assessments, securing community placements, and maintaining regular contact with clients, families, and facility staff.
