Case Manager

The Case Manager provides intensive, trauma-informed case management to individuals experiencing homelessness and housing instability, supporting programs such as The Bridge Program, Medical Respite, and Housing Support. This role involves conducting whole-person assessments, developing individualized care plans, and coordinating access to medical, behavioral health, and social services to promote long-term stability. The position requires collaboration with a multidisciplinary team to address patients' mental health, medical, and social determinants of health while advocating for their needs.
