Case Manager

The Case Manager provides intensive, person-centred support to young people aged 12 to 18 and their families in Mandurah, helping them navigate challenges such as family conflict, housing instability, and disengagement from education or community. The role involves building therapeutic relationships, conducting assessments, developing support plans, and connecting clients with essential services to promote safety, stability, and long-term wellbeing.
