Case Manager

The Case Manager works as part of a two-person field team with a law enforcement officer to engage individuals experiencing unsheltered homelessness, build trust through repeated outreach, and connect them to essential services, benefits, and housing. The role involves conducting assessments, navigating healthcare and housing systems, managing encampment resolutions, and maintaining detailed case documentation. This position requires strong relationship-building skills, crisis de-escalation abilities, and the capacity to collaborate effectively with city agencies and community partners.
