Case Worker CPHW

The Case Worker CPHW provides interpretation, translation, and culturally appropriate health education to patients in a comprehensive perinatal care program. The role involves conducting initial and trimester reassessments, developing individualized care plans, and coordinating obstetric, nutrition, and psychosocial services. The position also includes postpartum assessments, patient advocacy, and assistance with Medi-Cal applications.
