Client Care Navigator

The Client Care Navigator serves as the primary point of contact for clients with mental health and/or substance use disorders, providing outreach, screening, triage, and care coordination to ensure timely access to services. This role supports the full continuum of Certified Community Behavioral Health Clinic (CCBHC) services, including crisis care and outpatient treatment, while advocating for client needs and promoting seamless transitions. The position works closely with multidisciplinary teams to coordinate care from intake through ongoing treatment and connect clients with community resources.
