Client Navigator

The Client Navigator provides care coordination services to support timely access to appropriate levels of care, conducting screenings and risk assessments in alignment with CCBHC standards. The role facilitates referrals and linkage to internal and external medical, behavioral health, and community-based services to ensure continuity of care. Additionally, the position identifies and responds to urgent needs, including coordination with 24/7 crisis services, while maintaining compliance with state, federal, and organizational regulations.
