Clinical Pharmacist

The Clinical Pharmacist serves as an integral member of the interdisciplinary care team within the Care at Home Solutions program, optimizing medication therapy and improving clinical outcomes for individuals with complex medical, behavioral, and social needs. The role involves comprehensive medication management, including reconciliation, therapy management, and deprescribing, while collaborating with providers to enhance patient safety and reduce healthcare utilization. This part-time position supports core program pillars such as Annual Wellness Visits, Transitions of Care, and Complex Care Management.
