Patient Care Navigator

The Patient Care Navigator provides telephonic and field-based case management services to clients enrolled in the CALAIM Enhanced Care Management and Community Support Program, serving as the main point of contact to build strong relationships and promote adherence to medical care. This role focuses on removing barriers to care by identifying critical resources, facilitating navigation through healthcare systems, and collaborating with the Care Team to support positive client health outcomes. The position involves regular outreach, health education, and coordination with various healthcare providers to ensure comprehensive client support.
