Clinical Practice Pharmacist

The Clinical Practice Pharmacist provides advanced medication therapy management and direct or consultative patient care services across assigned practice areas within the Texas Department of Criminal Justice and Texas Juvenile Justice Department. This role integrates clinical decision-making with operational awareness to ensure safe, effective, and cost-conscious medication use while participating in interprofessional care teams. The position supports formulary management, clinical practice guidelines, and standardization of clinical pharmacy services across various practice settings.
