Clinical/Hospital Liaison

The role involves educating the community on rehabilitation services, identifying appropriate patients for transfer, and facilitating a smooth admission process. The position requires conducting pre-admission assessments, coordinating transfers, and maintaining strong relationships with healthcare professionals. Additionally, the liaison participates in community outreach and tracks key performance metrics.
