About this role
The Community Health Worker (CHW) is the Healthy Neighbor program’s frontline representative in the community, focused on improving patient health through social support, advocacy, and connection to care. This is a part-time, temporary onsite role based in Camden, NJ, engaging with patients at clinics, community sites, homes, and via remote outreach. The position emphasizes building relationships with diverse patients and linking them to clinical follow-up and community resources.
