Community Health Worker

The Community Health Worker supports Medi-Cal‑eligible adults involved in the justice system by providing outreach, care navigation, and non‑clinical case management to individuals transitioning from incarceration back into the community. The role ensures continuity of Medi‑Cal coverage, connects clients to health services and supportive resources, and promotes successful reentry to improve health outcomes and reduce recidivism.
