Medi-Cal Assessment Coordinator

The Medi-Cal Assessment Coordinator ensures participants receive timely assessment, enrollment assistance, and connection to Medi-Cal and other healthcare benefits to support successful community reentry. This role serves as a liaison between participants, correctional partners, and community agencies to facilitate continuity of medical and behavioral health services upon release from incarceration.
