Community Participation Facilitator

This role focuses on creating meaningful opportunities for individuals with disabilities to actively participate in their communities as valued neighbors and productive workers. The position involves close collaboration with stakeholders, including families and agency team members, to facilitate and coordinate activities that meet participants' needs. The role ensures the safety and comfort of all participants while managing external arrangements and accommodations. This is a part-time position requiring late afternoon and evening schedules, including alternating Saturdays.
