Discharge Planner

The Discharge Planner serves as a liaison between the inpatient setting and the assigned clinician at the Outpatient site, working with Level I Inpatient Providers to review clinical information and identify resources for successful discharge. This role involves monitoring follow-up with outpatient providers and collaborating with various stakeholders to ensure effective transitions to community services. The position aims to reduce readmission risks, decrease crisis service utilization, and improve coordination with primary care and psychiatric providers.
